Corpus Christi Catholic School requires authorization from a physcian and a parent/guardian to administer any medication (prescription or non-prescription) to students. If your child will be taking medication during school hours, please have your physician complete the Physicians’s Request Form contained in this handbook appendix. Please add your signature to this form and return it to school at the beginning of the new school year. The school nurse must have this form to allow your child to take any medication. The medication must be in a prescription container correctly labeled from the pharmacy. For over-the-counter medication, attach a label to the original container with the student’s name and dosage. The instructions on the label must match the doctor’s instructions on the prescription form. No medication can be dispensed without this documentation.
Please Note: School Health Services strongly recommends that the parent/guardian delivers medication to the school. Students may not have any prescription or non-prescription medication with him/her at any time.(See appendix for additional information)
